Billing Information

Baltimore Surgical Associates participates with most major insurance companies such as Medicare, BCBS, Cigna, United/Mamsi and Aetna. 

Please note the following:

• We ask that if your insurance company requires a referral, you contact your Primary Care Physician 48 hours prior to your appointment with our office when time allows.
• Patients are responsible for co-pays at the time of the visit.
• Coinsurance and deductibles are the patient’s responsibility.
• Patients must provide the correct and most recent insurance information. 
• If you have a procedure in our office-based Ambulatory Surgical Center(ASC), billing will be as follows:
• A professional fee for the surgeon performing the procedures and
• A separate facility fee.

Payment plans are available for uninsured or patients with high balances. We accept Visa, MasterCard and personal checks. We will obtain preauthorization and referrals for recommended surgical procedures.
